Brand Application Form - Applying for a new brand in North Dakota. 
**Please read instructions carefully prior to submitting an application.  Applications cannot be electronically transmitted, and all documents must be original forms.
 Copies are not accepted. 

Transfer Form -  Name changes, removing or adding a person (s), change of address, adding business to brand.
 

Bill of Sale - Use this form if you are the brand owner. If you are not the brand owner, a local inspection is needed.

The seller has 15 days to provide a bill of sale to the purchaser.

Affidavit for Collection of Personal Property -  To be used with a brand transfer form in the event a brand owner dies. 

Request for Removal of a Decedent - To be used with a brand transfer form n the event a brand owner dies.
